The Question is:
Hi! Alpha 4100 VMS V6.2-1H3. This customer4s machine has 4 SCSI adapters. One
of them (PKC) has a faulty disk. Since customer can4t stop the system rigth
now, he asked me a way to identify the SCSI adapter model (KZPSA, KZPAA
etc)without shutting down VMS
. From SDA I see PKC uses SYS$PKQDRIVER driver. From DCL I see only PK devices:

The OpenVMS Wizard is generally not in a position to provide hardware
support, nor to answer hardware-troubleshooting questions. (As was
mentioned in the introduction, the OpenVMS Wizard is familiar with
OpenVMS, but is generally not a StorageWorks, VAX, Alpha nor Itanium
hardware wizard.)
