Cause of DRVERR Fatal Drive Error?

The Question is:

 
I am trying to install a 4.3 GB SCSI drive in a Microvax 3100-80 machine and
 get the error "fatal drive error".  This drive came out of a novell server and
 had no issues there so I am operating under the assumption I don't know what I
 am doing!
 
I need to install this drive and initialize it in VAX....could you please
 provide instructions on how to do this?

  integration and testing.  You will also see one of the more common
