Bad or unsupported floppy disk causes crashes?

The Question is:

 
I have an alpha station 255 running Open VMS. Everything is working fine with
 the exception of writing from the floppy. I can read what's on the floppy, but
 can't write to the application drive. It will kick into BUGCHECK. This station
 did not come with a
 floppy, we inserted the floppy.
 
 

The Answer is :

 
  There is apparently either a hardware problem with the floppy disk,
  or you have an unsupported floppy disk.  Please contact your hardware
  services organization for assistance with this.
