The Answer is :

There can be no answer here, as there is insufficient information.

The particular symptoms reported imply a hardware problem or a core
  corruption of OpenVMS itself.

The OpenVMS Wizard will a request that you install the current ECOs
for V6.1 (an old release, and one that is no longer supported), or
move to OpenVMS V6.2 and ECOs (support available under Prior Version
Support contracts), or move to the current OpenVMS release and ECOs.

In particular, MOUNT was re-written for OpenVMS V6.2.

If you wish to pursue this, you will want to contact the Compaq
  Customer Support Center.
