DECforms FORMS TRANSLATE ACCVIO Error?

The Question is:

 
 
 When I try to compile the form  :
 
 FORMS TRANSLATE OMA_FORM -
 I get the following  Error  :
 
 %SYSTEM-F-ACCVIO, access violation,
 reason mask = 04, virtual address= 01441FC0,
 PC=00668C4C, PS=0000001B
 
 Improperly handled conditon, image exit forced
 
 & Register dump's below
 
 Pl advice how to gt out of this problem.

  Please contact the Compaq Customer Support Center directly, as
  resolution of this problem will likely involve the DECforms
  version information and access to the target DECforms source
