Errors on secondary key in RMS index file?

The Question is:

 
When trying to read by a secondary key in an ISAM file, programs frequently
 return an error even though the key is in the file.  The file converts without
 errors.  Also, if the program is executed again, it may succeed in reading by
 the key.  I am using O
penVMS 7.1-1H2.  What could be causing this problem?

  Please contact the Compaq Customer Support Center, as details on
  the particular error(s) encountered will be required.  Potentially,
  a copy of the errant RMS indexed file and/or the source code of a
